As parent and/or guardian of my registered child, a minor, I hereby authorize the treatment by a qualified and licensed medical doctor in the event of a medical emergency which, in the opinion of the attending physician, may endanger my child’s life, cause disfigurement, physical impairment or undue discomfort if delayed. This authority is granted only after a reasonable effort has been made to reach me. I further authorize that my child may be transported to a hospital or emergency clinic for treatment. The dates during which this release is granted is from March 10, 2024 to June 30th, 2024. The information provided above (family physician, allergies/illnesses/conditions, and emergency contact info) is completed of my own free will for the sole purpose of authorizing medical treatment under emergency circumstances in my absence.  I hereby agree that my registered child(ren) and myself have carefully read, fully understand, and agree to the code of conduct set forth by the Diocesan of Trenton.

In consideration of the acceptance of my registration, I, for myself, my executors, administrators and assigns do hereby release St. Charles Borromeo School and Parish, RunSignup, all volunteers, owners of fields and facilities utilized by this program, all sponsors and organizers from all claims, demands, suits or actions whatsoever in any manner for injuries or damages I may sustain as a result of my participation. I attest I have full knowledge of the strenuous nature of this activity and am medically able to participate and I am willing to assume all risks involved. I also give permission for free use of my name and picture in any broadcast, release, social media, or print media account of the St. Charles Borromeo Track Program. I also give permission for my e-mail address and mobile phone to be sent messages related to the St. Charles Borromeo Track Program. By submitting this participation, I acknowledge (or a parent or adult guardian for all children under 18 years) having read and agreed to the above waiver.
