I have read and understand this release of liability. I, freely accept and voluntarily assume any and all risks associated with my pet, myself, and unregistered individuals in my party participating in this race, including all activities, prior to and after the race, including but not limited to injuries, falls, contact with other participants and their pets, traffic risks, effects of weather, and illness. I know and understand that participating in the race is a potentially hazardous activity and I know and appreciate all risks associated with this activity, including risks of serious injury and/or death. In consideration of being allowed to participate in the race, I hereby for myself, heirs, executors, administrators, and anybody else who could make a claim by or through me or on my behalf, agree not to sue and waive, release and discharge the Animal Rescue League of New Hampshire (ARLNH), Second Wind Race Timing, all sponsors, volunteers, race officials/directors, vendors and anybody else who could be held liable by or through the Animal Rescue League of New Hampshire from any and all claims of liability, including but not limited to claims of negligence, for death, personal injury, or damage of any kind or nature arising out of my/my pet’s participation in this event, including participation in any activities prior to or after the race. This release and waiver extend to all claims of every nature whatsoever, foreseen, known, or unknown, including negligence. Further, I hereby grant full permission to any and all of the foregoing to use my likeness in all media including, but not limited to photographs, broadcasts, newspapers, brochures, or any other record of this event for any legitimate purpose without compensation. Athlete acknowledges that the entry fee paid is non-refundable and non-transferable. Athlete acknowledges and agrees that Howl-O-Ween 5k and ARLNH, in its sole discretion, may delay or cancel the Event if it believes the conditions on the race day are unsafe. I understand that all pets must remain on-leash at all times, be up-to-date on all vaccinations and be pest free. I HAVE READ THIS RELEASE, UNDERSTAND IT AND SIGN IT VOLUNTARILY. IF ATHLETE IS UNDER AGE 18 HIS/HER PARENT OR GUARDIAN MUST SIGN THIS RELEASE

AND WAIVER AGREEMENT. Athlete's Parent or Guardian's signature above certifies that my son/daughter/ward has my permission to participate in the Event. Athlete's Parent/Guardian has read and understands the foregoing RELEASE AND WAIVER OF LIABILITY AGREEMENT (above) and by signing intentionally and voluntarily agrees to its terms and conditions. Athlete's Parent/Guardian further certifies that my son/daughter/ward is in good physical condition and is able to safely participate in the Event. I hereby authorize medical treatment for him/her and grant access to my child's medical records as necessary and as stated above
