In consideration of the right to participate in the 2022 Meriden Daffodil Dash 5K Road Race & Kids Fun Run, I do hereby, for myself, my heirs and executors, waive and release forever and discharge any and all claims for damages which I may have or which may hereafter accrue to me against all sponsors including their agents, race officials, the Meriden Parks & Recreation Department, the Meriden Running Club, the City of Meriden, United States of America Track and Field (USATF) and their officers, agents, assigners, and volunteers. I further certify that I am in satisfactory physical condition and have made ample preparation for running a 5K or have had a recent physical exam by a certified doctor. By submitting this entry, I acknowledge (or a parent or adult guardian for all children under 18 years) having read and agreed to the above release and waiver. Further, I grant permission to all the foregoing to use my name, voice and images of myself in any photographs, motion pictures, results, publications or any other print, videographic or electronic recording of this event for legitimate purposes.
