I acknowledge that this event is a test of a person’s physical fitness and limits and carries with it the potential for death, disability, personal injury, property damage, and property theft. I attest and verify that I (or the person I am registering) am physically fit and sufficiently trained for the completion of these events and my physical condition has been verified by a licensed medical person, and I assume all risks, including but not limited to falls, weather, traffic and course conditions.

In submitting my entry, I, for myself, my heirs, executors and administrators, do hereby waive, release, discharge and covenant not to sue any of the following entities: the Whitefish Bay Public Education Foundation, the Village of Whitefish Bay, Silver Circle Sports Events, LLC, any agency thereof, all sponsors, all service and product providers, and their employees, officers directors, principals, agents, representatives, successors and assigns and any and all event volunteers (known for all purposes herein referred to as “Releasees,”) from any and all liability for my death, disability, personal injury, property damage, property theft or actions of any kind which may hereafter accrue to me as a result of my participation in this event or my traveling to and from this event, arising out of unintentional, accidental, negligent acts or other omissions of the Releasees.

In submitting my entry, I, for myself, my heirs, executors and administrators, do hereby indemnify and hold harmless the following entities: the Whitefish Bay Public Education Foundation, the Village of Whitefish Bay, any agency thereof, all sponsors, all service and product providers, and their employees, officers directors, principals, agents, representatives, successors and assigns and any and all event volunteers from any and all liabilities or claims made by other individuals or entities as a result of my actions during this event.

I hereby grant full permission to any and all of the foregoing to use my name, my voice, my picture and/or my likeness in any photograph, publication, broadcast, electronic recording, advertising, promotion or other account of these events for any purpose. I understand that the entry fee is nontransferable and nonrefundable.

By submitting this entry, I acknowledge (or a parent or adult guardian for all children under 18 years) having read and agreed to the above release and waiver.
