To reserve a vendor booth online: Click here and you will be prompted to create an account and provide your company name and high-resolution logo. An invoice will be sent and payments can be made online or by mail.
To reserve a vendor booth by mail: Download and send completed Participation Form to the Lions Eye Institute Foundation, 1410 North 21st Street, Tampa, FL 33605, Attn: Holly Moon. Reservation requests must be received no later than Monday, March 25, 2019.
Offline Payments: Checks can be made payable to the Lions Eye Institute Foundation and should be mailed to: 1410 North 21st Street, Tampa, FL 33605, Attn: Holly Moon.
