Please read this Agreement carefully as it impacts your legal rights.

I, the undersigned, wish to participate in the Fulton County School event or activity as referenced above (hereinafter referred to as “Activity or Event”). The
individual participating in the Event is hereinafter referred to as the “Staff Member/Participant” as indicated above. I understand that participation in the
Event is completely voluntary, Staff Member/Participant is not obligated to participate, and Staff Member/Participant will not receive a benefit from
participation other than the opportunity to participate in the Event.

I understand and acknowledge that this Event by its very nature poses actual or potential risks of physical and emotional injury/illness, including but
not limited to death, to Staff Member/Participant or to any individual who participates in such Event. I am aware that there may be no Fulton County
School District (“District”) insurance that would provide coverage to Staff Member/Participant for medical treatment, personal injuries, or property
damage which may arise out of this Event.

In order to participate in this Event, I agree, to the fullest extent allowed by law, to assume all liability and responsibility for any and
all potential or real risks, injuries, or even death which may result from participation in the Event. I represent and warrant that (a)
Staff Member/Participant is mentally and physically fit, capable, able, and willing to participate in this Event without limitation, and
(b) Staff Member/Participant does not suffer from any physical or psychological conditions that would prevent Staff
Member/Participant’s safe participation in the Event. I agree to follow or require Staff Member/Participant to follow all safety
precautions and instructions for the Event.

To the fullest extent allowed by law, I understand, acknowledge, and agree that the District, the Fulton County Board of Education,
and the past, present, and future officers, members (including Fulton County Board of Education Board Members), attorneys, agents,
employees, predecessors, successors in interest, and assigns of the District and Fulton County Board of Education (collectively,
“District Parties”) shall not be liable for any injury, illness, or death suffered by Staff Member/Participant which arises out of and/or
is associated with preparing for or participating in the Event, and I release, to the fullest extent allowed by law, the District Parties
from all such liability.

To the fullest extent allowed by law, I hereby release, discharge, indemnify, and agree to hold harmless the District Parties from any
and all liability arising out of or in connection to Staff Member/Participant’s participation in the Event. For purposes of this
Agreement, “liability” means all claims, demands, losses, causes of action, suits, or judgments, including attorneys’ fees, of any
kind that Staff Member/Participant or Staff Member/Participant’s parents, guardians, relatives, heirs, executors, administrators,
and assigns have or may have against the District Parties due to Staff Member/Participant’s personal, physical, or emotional
injury, accident, illness or death, or because of any loss of or damage to property resulting to Staff Member/Participant or his or her
property during or as a result of Staff Member/Participant’s participation in the Event due to the acts of anyone including District
Parties, including acts of passive or active negligence by District Parties other than actions involving fraud or actual malice.

Staff members/Participants may be included in public relation activities, events, and/or publications. I consent to the District’s (and its photographers’)
use of Staff Member/Participant’s name, picture, likeness, work, voice, or verbal statement to appear in publicity, publications, videos, websites, or any
other media. I understand and agree that (a) no monetary consideration shall be paid for such use; (b) my consent and release have been given without
coercion or duress; and (c) Staff Member/Participant’s picture, likeness, work, voice, or verbal statement may be used in subsequent years.

A signed Agreement must be on file with the District before Staff Member/Participant will be allowed to participate in the Event. Staff Members/Participants
who do not wish to accept the risks described in this Agreement should not sign this Agreement and will not be allowed to participate in the Event.
I acknowledge that I have carefully read this Agreement and that I understand the potential dangers of engaging in this Event, am fully aware of the
legal consequences of this Agreement and agree to its terms. I understand I am waiving certain rights and assuming the risk of injury, including
death and property damage, from my participation in the Event. This Agreement shall be governed by the laws of the State of Georgia. Any suit
arising from this Agreement, or the Event shall be brought in the State or Superior Courts of Fulton County, Georgia. The terms of this Agreement are
severable; if any clause is found to be invalid, the offending clause will be stricken, and the balance of the Agreement will remain enforceable and in effect.
This Agreement shall remain in full force and effect throughout and following the Event and shall survive the end of the Event.
