I acknowledge that participation in Centerstone's Life;Story events is a potentially hazardous activity, which could cause injury or death. I attest that I am medically able to participate and have properly trained, I agree to abide by any decision of a race official relative to any aspect of my participation in this event, including the right of any official to deny or suspend my participation for any reason whatsoever. I attest that I have read the rules of the race and agree to abide by them.  I assume all risks associated with participation in this event, including but not limited to: falls, physical contact with other participants, volunteers, race personnel, contract service providers, employees, and spectators including the potential the contraction of a communicable disease resulting from contact with other participants, volunteers, race personnel, contract service providers, employees, and spectators.  In addition, I assume all risks including: the effects of the weather; high heat and/or humidity; freezing cold temperatures; traffic and the conditions of the road including surrounding terrain

I hereby certify the following: (1) I am physically fit and received medical clearance to participate in Centerstone's  Life;Story events. (2) In consideration for my, or my child's application and acceptance for participation in the Life;Story events. On behalf of myself, my heirs and assigns and my estate, hereby release and forever discharge the sponsors, organizers, vendors, race directors, affiliates as well as their agents and employers from any and all claims that may accrue as a result of my participation in this athletic contest and affirm that I understand that this release includes claims based on negligence, action or inaction of any of the above released parties and covers bodily injury and property damage, whether suffered by the registered participant during or after such participation. (3) I hereby grant Centerstone specific permission to reproduce, publish, circulate, copy write or otherwise use all photographs and/or video tape of me and/or my family taken at the Centerstone Life;Story events and for use by Centerstone, the contracted Race Management company, and the contracted Photographer. Further, I hereby grant Centerstone, the contracted Race Management company, and the contracted Photographer permission to publish other records of my participation in Life;Story events including, but not limited to, my name, gender, age, domicile, and race results.

I waive any right to inspect or approve any photo, video, or audio recording. I have been fully informed of my consent, waiver of liability, and release before participating in this event.
