ACKNOWLEDGEMENT OF RISK AND WAIVER OF LIABILITY

A waiver is required for all participants. Adults may not sign for other adults.  

A parent or guardian must sign for any participant under 18 years old.  

READ BEFORE ACKNOWLEDGING.

I know that participating in the Walk in the Park for Brain Tumor Research is a potentially hazardous activity and I should not enter or participate unless I am medically able. I acknowledge and assume any and all risks associated with the event, including but not limited to traffic on the course route, falls, contact with other participants, and the condition of the course, including but not limited to, curbs, cars, uneven pavement, rocks and other objects on the course surface. Knowing and appreciating these risks and in consideration of Esky Walk in the Park’s acceptance of my entry(ies) I hereby, on behalf of myself and all minors listed above, as well as all heirs and assigns thereof, covenant not to sue and waive, release and discharge Esky Walk in the Park, Upper Michigan Brain Tumor Center, Superior Health Foundation, RunSignup.com, City of Escanaba and any volunteers, sponsors, organizers, officers, agents and employees of the aforesaid from any and all claims, demands, damages, actions, or rights of action of any kind or nature, including but not limited to injuries to person or damages to property, arising out of or in any way related to my participation in this event.  I also grant permission to Esky Walk in the Park to use any photographs, motion pictures, recordings or other records of this event for any legitimate purposes.

My signature acknowledges that I have read the above waiver and agree and accept all terms and conditions set forth therein.
