In consideration of the acceptance of my entry to participate in the World Race for Hope 5k charity run/walk (the "Event"), I hereby for myself, my heirs, and my personal representatives assume any and all risks that might be associated with the Event. I hereby release the Waste-Free Dayton, the City of Troy, Ohio, RunSignUp.com, as well as their representatives, employees, officials, volunteers, successors and assigns for any and all injuries and damage to me or my property in said Event, and agree to hold the above harmless from any claims. I understand that all registration forms for participants under 18 years of age must be signed by a parent or legal guardian. I understand that I am solely responsible for my health and safety in connection with the Event, and I acknowledge that I am physically capable of participating in and completing the Event. I agree that I am volunteering to participate in the Event and understand that I will not receive any payment whatsoever for my voluntary participation. I further give my permission for the free use of my name, likeness or voice in the use of any photo, film or videotape of the Event for any purpose.

By submitting this entry, I acknowledge (or a parent or adult guardian for all children under 18 years) having read and agreed to the above waiver.
