PLEASE READ THIS DOCUMENT CAREFULLY. BY SIGNING THIS DOCUMENT,
YOU WAIVE IMPORTANT LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE.
I, the undersigned, wish to participate in the Strut with Your Mutt 5K (5K or 1 Mile distance,
run or walk) event (hereinafter “Event” located at The English Country Barn of the Carolinas. I
acknowledge the event may result in personal injury to me, any minor under my care, and/or
my dog(s) (Dog). I understand that participation in this Event is entirely voluntary and
involves certain risks, including but not limited to injury to myself, my dog, other participants,
and third parties.
In consideration of being allowed to participate in the Event, I, for myself, my heirs,
executors, and assigns, agree as follows:
1. ASSUMPTION OF RISK
I acknowledge that participation in the Event may include hazards that are incidental to or in
any arise from the event including without limitation, all risks and danger such as: Canine
oriented event risks (e.g. dog bits, fights, or altercations, falls, and contact with participants or
spectators at the event), collisions or altercations with other participants, dogs, vehicles, or
obstacles; exposure to extreme temperatures, humidity, or weather conditions; my dog’s
behavior and interactions with other animals and participants; conditions of the course
including but not limited to uneven terrain, slips, trips, or falls, risks from the sport of walking
and running. I acknowledge that walking a dog is a potentially hazardous activity and I should
not walk or run unless I am medically able to. I understand that walking and running in races
are potentially hazardous activities which could cause injuries or death. I should not enter and
run or walk in the event unless I am medically able and property trained, and by my signature, I
certify that I am medically able to perform this event, and am in good health, and I am properly
trained. I consent to emergency medical care and transportation in the event of injury and to
obtain treatment medical professionals may deem appropriate. I voluntarily assume full
responsibility for any risks, injuries, or damages (including but not limited to physical injury,
illness, or death) arising from participation in the Event.
2. RELEASE AND WAIVER OF LIABILITY
I hereby release, waive, and discharge Adoption First Animal Rescue, Bluedunes LLC, the Event
organizers, sponsors, volunteers, vendors, venue property owners, and their officers,
employees, agents, and affiliates (collectively, “Released Parties”) from any and all claims,
liabilities, or damages that may arise from my participation in the Event, even if caused by the
negligence of the Released Parties.
3. INDEMNIFICATION
I agree to indemnify and hold harmless the Released Parties from any claims, demands, losses,
or damages arising out of my or my dog’s actions, including injury to other participants, dogs,
spectators, or property damage.

Is up-to-date on all vaccinations, including rabies; is in good health, not aggressive, and
well-socialized with humans and dogs; the Dog is not the subject of a quarantine order, a
pending adjudication or administrative hearing, and has never been declared a dangerous,
potentially dangerous or vicious or potentially vicious dog. Will remain on a secure 6-foot
leash (no retractable leashes) at all times. I acknowledge and understand that any dog showing
signs of aggression, fear, or stress will be asked to leave the Event, with no refund provided. I
understand I shall be fully responsible for any damages or expenses, direct or indirect, incurred
as a result of myself or my Dog's disruptive behavior and/ors ejection. I further agree that any
event official may authorize necessary treatment for me and or the Dog in the event of injury.
5. EVENT RULES
I agree to: clean up after my dog immediately; ensure that my dog is under my control and
supervision at all times; Follow all Event rules and instructions from staff or volunteers. No
child under the age of 12 will be handling a dog during the Event.
6. PHOTO AND MEDIA RELEASE
I grant permission to all of the foregoing to use my photographs, motion pictures, recordings or
any other record of this event for any legitimate purpose.
7. ACKNOWLEDGMENT OF RELEASE
I understand that this Waiver and Release is legally binding and that I have read it in its
entirety. By signing below, I agree to its terms voluntarily and with full understanding of its
legal consequences.
