I hereby acknowledge, agree and understand that the Walk Like MADD®, MADD Dash, MADD Monster Mash & Dash, Golf Like MADD, Fish Like MADD, Ride Like MADD and any other related MADD fundraising events (each a “MADD Event” and collectively, the “MADD Events”) involves walking, running and, in some instances golfing, fishing, swimming, cycling and even motorcycling – all activities that may include risks such as, but not limited to, falls, crashes, interaction with other participants (including the potential transfer of communicable diseases between participants), effects of weather, traffic and conditions of the road and waterways. I further understand that MADD is not responsible for any loss, damage, or theft of bicycles or other personal belongings that may occur as a result of participation in the MADD Event. In consideration of being allowed to participate in this MADD Event, I hereby expressly assume all risks, including personal injury and death, arising in any way out of my participation in the MADD Event and related activities. 
It is my responsibility to dress appropriately including, in some instances, wearing protective gear including a helmet. Although route maps, rest stops, refreshments, and other assistance may be made available during the MADD Event, I am solely responsible for my own health and safety. I represent and warrant that I am physically fit and able to participate in this MADD Event and I agree to stop and request assistance if I experience any symptoms such as, but not limited to, dizziness, excessive fatigue, shortness of breath, pain or any other conditions which would make it difficult or unsafe to continue. 
I agree, for myself, my heirs, executors and administrators, to not sue and to release, indemnify and hold harmless Mothers Against Drunk Driving (“MADD”), its affiliates, officers, directors, employees, contractors, volunteers, and representatives, and all sponsoring businesses and organizations and their agents and employees, from any and all liability, claims, demands, and causes of action whatsoever, arising out of my participation in the MADD Event and related activities - whether it results from the negligence of any of the above or from any other cause. 
This release and indemnification agreement (“Agreement”) shall be as broad and inclusive as is permitted by the State or Province in which the MADD Event is conducted. If any portion of it is held invalid, the balance shall continue in full force and effect. I have read, understand and agree to the terms of this Agreement. If Participant is a minor, the parent or guardian must agree to the below: I am the legal guardian of Participant, and I hereby consent to his/her participation. 
I understand that the registration fee is non-refundable/non-transferrable.   
I also understand that depending on the venue of the MADD Event, I may also be asked to sign the venue’s waiver, release and indemnification form in addition to this waiver. 
For consideration received, I hereby give permission without restrictions to MADD, its successors and assignees to use my name, likeness, pictures, video and/or voice in connection with still photography, in print materials, a promotional video or any other purpose in furtherance of MADD's mission.  My consent is granted with the understanding that MADD has the sole discretion to cut and edit the film, video or voice recording of my appearance. I specifically waive the rights of privacy, publicity or any other rights I may have.

Animals at MADD Events 
In the event that I bring my pet to the MADD Event, if allowed, I agree to indemnify, defend and hold MADD, its officers, employees, directors, contractors, volunteers, participants, and others in attendance at the MADD Event harmless from any damages, injuries, liability, suits, fees, and expenses associated with any actions of my pet. My pet and his/her behavior is solely my responsibility, and I agree to assume full responsibility for any damages or losses caused by the same. I agree that any pet I bring to the MADD Event will have all current vaccinations and/or shots required for their species, and that at all times during attendance at the MADD event I shall keep my pet on a leash. I further understand and agree that aggressive pets or pets that are otherwise acting in an aggressive manner are not permitted at the MADD Event. I understand that I must leave the MADD Event if at any time any MADD employee, agent, contractor or volunteer asks me to leave if they believe (in their sole discretion) that I have violated these rules regarding pet attendance at the MADD Event. 
Minor Participants 
If Participant is a minor, the parent or guardian must agree to the below: 
I represent and warrant that (a) I am the legal guardian of the minor named as the Participant, (b) no court has issued any order, judgment, or decree granting custody of the minor to anyone else or otherwise affecting my rights as parent or legal guardian, (c) the minor has not been emancipated, (d) I have the legal right, power, and authority to consent to this Agreement on behalf of the minor and myself, and (e) I am at least 18 years of age. I have read, and I understand, this entire Agreement. By signing below, I hereby consent to MADD’s use of the minor’s likeness, pictures, video and/or voice in connection with still photography, in print materials, a promotional video or any other purpose in furtherance of MADD’s charitable mission.  My consent is granted with the understanding that MADD has the sole discretion to cut and edit the film, video or voice recording of my minor child.  I specifically waive the rights of privacy, publicity or any other rights of the participating minor child at the MADD Event.
