2024 CureFest Waiver of Claim

TERMS OF RELEASE OF LIABILITY & HOLD HARMLESS

I will participate in the CureFest. I am 18 years or older and provide my consent for me
and any child of mine under 18 accompanying me.

I hereby release, discharge and hold harmless the above parties, those acting pursuant to their
authority, together with their successors and assigns from any liability or claim that I may have
against CureFest organizers or the venue and its staff with respect to bodily injury, personal injury,
illness, death, or property damage that may result from my involvement, or that of my minor child,
in CureFest activities. I release and hold harmless the aforenamed from liability, claims, and demands
of whatever kind or nature, in law or in equity, which arise or may hereafter arise, from the activities
of CureFest, including claims arising out of negligence, for any violation of any personal or proprietary
right I may have in connection with my image or voice, including any and all claims for libel, defamation,
or invasion of privacy. I understand that all such recordings, in whatever medium, shall remain the
property of the parties, and any one of them listed above. I have read and fully understand the terms
of this release. I agree to hold harmless and indemnify said parties from claims brought by me or on
my behalf concerning such production or use.

By submitting this entry, I acknowledge (or a parent or adult guardian for all children under 18 years) having
read and agreed to the above waiver.
