Release of Liability:
I understand that I am voluntarily participating in any activity at any of the amenities or event organized by Treasure Lake Property Owners Association, Inc.
I acknowledge and understand that the activity or event involves certain risks and hazards. I voluntarily assume all risks associated with my participation, including but not limited to accidents, injuries, property damage, and any other risks inherited in the activity or event.

In consideration of being allowed to participate in the activity or event, I hereby release, discharge and hold harmless to Treasure Lake Property Owners Association, Inc. and its directors, officers, employees, instructors, volunteers, partners and any person representing on behalf of Treasure Lake Property Owners
Association, Inc., and runsignup.com from any and all claims, demands, actions, causes of action, liabilities, costs, expenses, or damages of any kind, whether know or unknown, that may arise out of or in connection with my participation.

I hereby represent that I am physically fit and have no medical conditions that would prevent my full participation in the activity or event. If there are any changes to my health or fitness status that may affect my ability to participate, I agree to notify Treasure Lake Property Owners Association, Inc promptly. In the event of any injury or medical condition arising during my participation in the activity or event. I authorize the Treasure Lake Property Owners Association, Inc to obtain medical treatment on my behalf, including but not limited to emergency medical care, hospitalization, surgery administration of medication. I agree to be
responsible for any costs associated with such treatment. 

I understand that Treasure Lake Property Owners Association, Inc does not provide personal insurance coverage for participants. I acknowledge that I am responsible for obtaining and maintaining any necessary insurance coverage, including but not limited to health insurance to protect myself during my participation. 

I agree to comply with all rules, regulations, instructions and guidelines provided by Treasure Lake Property Owners Association, Inc regarding activities, events or use of amenities.  

Event Photography Release:
I understand that any event that I register for that is organized or sponsored by Treasure Lake Property Owners, Inc will have photography taken of the event. 
I hereby authorize Treasure Lake Property Owners Association, Inc to edit, change, copy and make any use of photographs, images, and videos for promotional materials.

I hereby acknowledge that I will not be entitled to payment or any sort of compensation for such action. I authorize the use of my photograph, image, or video for, but not limited to, publication on the internet, magazines, journals, books, articles etc., provided it is done for a lawful purpose. Upon the usage of my photographs, images, and videos, I consent to such materials becoming the sole property of Treasure Lake Property Owners Association, Inc. and that I will no longer be entitled to them, provided it is done for lawful purposes. I hereby release all rights to any, but not limited to, claims, rights, demands, and/or any cause of action by me or my representatives, heirs, or anyone else.

Furthermore, I hereby waive my right to any royalty or any other compensation with regard to the usage of the photos, images, or vides referred to in this form.

By submitting this entry, I acknowledge (or a parent or adult guardian for all children under 18 years) having read and agreed to the above release and waiver including the no refund policy.
