I/we hereby acknowledge and agree that in consideration of being permitted to participate in a "Ranger Run 5K" event (hereinafter referred to as the "Event") organized and operated by Deerpark Middle School PTA (hereinafter referred to as the "Event Organization"):

1. I/we do hereby release the Event Organization, its members, officers, managers, directors, employees, independent contractors, agents, affiliated clubs, and volunteers from all liability, recourse, proceedings, claims, and causes of action of any kind whatsoever, in respect of all personal injuries or property losses which I/we may suffer arising out of our participation in, the Event, notwithstanding that such injuries or losses may have been caused solely or partly by the negligence of the Event Organization, or any of its members, officers, managers, directors, employees, independent contractors, agents, affiliated clubs, and volunteers. I/we understand that riding is always prohibited in the venue and campground areas and that I will dismount before entering such areas.
2. I/we understand that alcohol is not allowed in the venue.
3. I/we understand that Deerpark Middle School PTA/event operations has the right to text me/us during the event regarding emergency situations.
4. And, I/we hereby acknowledge and agree:
   1. that the Event exposes participants to many dangers, risks and hazards, some of which are inherent in the very nature of the Event itself, others which result from human error and negligence on the part of the persons involved in preparing, organizing, and staging the Event and other activities, including without limitation the dangers of collision with pedestrians, vehicles, other racers, and fixed or moving objects; the dangers arising from surface hazards, equipment failure, inadequate safety equipment, THE UNDERSIGNED'S OWN NEGLIGENCE, and weather conditions; and the possibility of serious physical and/or mental trauma or injury associated with activities in the Event;
   2. that, as a result of the aforesaid risks and hazards, I/we may suffer serious personal injury, even death, as well as property loss;
   3. that some of the aforesaid risks and hazards are foreseeable, but others are not;
   4. that I/we nevertheless freely and voluntarily assume at the aforesaid risks and hazards, and that, accordingly, my/our participation in the Event shall be entirely at my/our own risk;
   5. that I/we understand that neither the Event Organization nor any of its members, officers, managers, directors, employees, independent contractors, agents, affiliated clubs or volunteers assume any responsibility whatsoever for my safety during the course of my participation in the Event;
   6. that I/we have carefully read this RELEASE, WAIVER, AND ASSUMPTION OF RISK agreement, that I/we fully understand same, and that I/we am/are freely and voluntarily executing same;
   7. that I/we understand clearly that by accepting this release I/we will be forever prevented from suing or otherwise claiming against the Event Organization, its members, officers, managers, directors, employees, independent contractors, agents, affiliated clubs or volunteers for any loss or damage connected with any property loss or personal injury that I/we may sustain while participating in the Event whether or not such loss or injury is caused solely or partly by the negligence of the Event Organization or any of the its members, officers, managers, directors, employees, independent contractors, agents, affiliated clubs or volunteers;
   8. that I/we understand that at this event or related activities I/we may be photographed, and that I/we agree to allow my photo, video, and/or film likeness to be used for any legitimate purpose by the Event Organization, producers, sponsors, and/or assigns;
   9. that I/we have been given the opportunity and have made a personal decision whether to seek independent legal advice prior to signing this agreement;
   10.  that I/we consent to receive medical treatment, which may be deemed advisable in the event of injury, accident, and/or illness during the event;
   11. that I/we understand clearly that the Event Organization will not permit me to participate in the Event unless I/we accept this RELEASE, WAIVER, AND ASSUMPTION OF RISK AGREEMENT, and that the terms of this Agreement need not be brought to my/our attention by the Event Organization at any time after I/we accept this Agreement;
   12. that the term "accept" in this Agreement includes executing a signature or any other positive action of agreement in writing or electronically;
   13. that the use of the terms "I/we," "my/our," and variations thereof in this RELEASE, WAIVER, AND ASSUMPTION OF RISK AGREEMENT indicates that I have been given express authorization by the other participants in my team, which is entered into the Event including all of the names of the members of the team, to act on their behalf in accepting this Agreement, and I have given them copies of this Agreement for their approval before such authorization is given; and
   14. that this RELEASE, WAIVER, AND ASSUMPTION OF RISK agreement is binding on myself/ourselves, my/our heirs, my/our executors, administrators, personal representatives and assigns

Minors For each participant under the age of 18, this Agreement must also be executed by a parent or guardian. Parent or Guardian Volunteer Waiver For Minors (Under 18 years old) The undersigned parent and natural guardian or legal guardian does hereby represent that he/she is, in fact, acting in such capacity and agrees to save and hold harmless and indemnify each and all of the parties referred to above from all liability, loss, cost, claim, or damage whatsoever that may be imposed upon said parties because of any defect in or lack of such capacity to so act and release said parties on behalf of the minor and the parents or legal guardian.
