I wish to participate in the May 3, 2025 Walk and Roll, a benefit for the Wilson Research Foundation at Methodist Rehabilitation Center (“Event”). I understand the acceptance of the waiver is required to participate.

I further understand that I am solely responsible for my health and safety, and I acknowledge that I am physically capable of participating. In consideration of my participation in this Event, I, for myself and my heirs, release and forever waive any and all claims, actions, damages, or loss (including the loss of personal items), that are related to or are in any way connected to my attendance or participation in any of the activities associated with this Event that I may have against the Wilson Research Foundation, Methodist Rehabilitation Center and/or any of its employees, agents or any other event organizers or sponsors who may be affiliated with this Event. 

If I bring a pet to this event, I assume full responsibility and will leash and control the animal at all times. I am aware that there will be a mobile Petting Zoo at the event and that other pets brought by guests are prohibited from being anywhere near the Petting Zoo.  I AGREE TO KEEP MY ANIMAL AWAY FROM THE PETTING ZOO (if applicable.) 

I agree to allow Wilson Research Foundation, and their contractors, agencies and sponsors, the use of my name and likeness in connection with this event, for any purpose related to advertising or promotion of the event worldwide in perpetuity in all forms of media.

I am aware that this is a RELEASE OF LIABILITY and a contract between me and the persons and entities mentioned above and that I accept of my own free will.

We reserve the right to postpone or cancel the event due to circumstances beyond our control such as weather, a natural disaster or emergency or as required to protect the safety of participants and staff. 

By submitting this entry, I acknowledge (or a parent or adult guardian for all children under 18 years) having read and agreed to the above release and waiver.
