Assumption of Risk
I understand that participation in a 5K run/walk and fun run involves inherent risks, including but not limited to: falls, contact with other participants, the effects of weather (heat, humidity, rain), traffic, and conditions of the road or trail. I knowingly and freely assume all risks associated with my participation.

Release of Liability
I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless the Mena/Polk County Chamber Enhancement Foundation, its board, officers, employees, volunteers, sponsors, and partners from any and all claims, demands, or causes of action arising out of my participation, including injury, illness, or property damage.

Medical Consent
In the event of an emergency, I authorize the Event organizers to secure medical treatment on my behalf. I assume responsibility for any medical expenses incurred.

Fitness to Participate
I certify that I am physically fit to participate and have not been advised otherwise by a qualified medical professional.

Code of Conduct
I agree to follow all instructions from race officials, volunteers, and law enforcement for my safety and the safety of others.

Publicity Release
I grant permission for photos, video, or audio recordings taken of me during the Event to be used by the Mena/Polk County Chamber Enhancement Foundation for promotional purposes without compensation.

Acknowledgement
I certify that I have read this waiver and release, understand its terms, and agree to them voluntarily.
