I am submitting this application with the intent to obtain an honorary cannonball and accompanying plaque for permanent mounting on the U.S. 250th Combat Veterans Memorial to forever honor a U.S. Combat Veteran. 

I understand that the acceptance of my application is contingent on my submitting a DD214 form or other form of verification that proves the Veteran to be honored served the U.S. in combat, and received an honorable discharge (if applicable).

I understand honorary cannonballs will be issued on a first-come-first-served basis from among approved applications.

Accepted applicants will be contacted by a project coordinator and provided secure means for making a $750 tax-deductible donation to obtain an honorary cannonball.  Donors will receive an electronic receipt for their donation.

I understand project coordinators may correct spelling and punctuation errors on this application without contacting the applicant.  If inaccuracies are detected or too many characters appear on a line of text in a submission, applicants may be contacted by a project coordinator if the application is otherwise acceptable.

I understand that currently there is no set time for the groundbreaking or completion of the memorial, but project coordinators and the City of Whitehall warrant that they are putting forth their best efforts to complete the memorial in a timely fashion.  Once a donor's cannonball and accompanying plaque are mounted on the memorial, a photo file of the displayed cannonball and plaque, along with a number for locating it on the memorial, will be sent to the donor.
