I know that participating in a running race is a potentially hazardous activity, which could cause injury or death. I certify that: I am medically able to participate in this race; am in good health; and am properly trained. I agree to abide by any decision of a race official relative to any aspect of my participation in this event, including the decision of any official to deny or suspend my participation for any reason whatsoever. I attest that I have read the rules of the race and agree to abide by them. I assume all risks associated with participating in this event, including, but not limited to: falls and physical contact with vehicles, bicycles, other participants, volunteers, race personnel, service providers, employees, and spectators and the contraction of a communicable disease resulting from contact with other participants, volunteers, race personnel, service providers, employees, and spectators. I further assume the risk of: the effects of the weather; high heat and/or humidity; cold temperatures; and traffic and the conditions of the road, trail, greenway, or track, including surrounding terrain.

I understand that bicycles, skateboards, roller skates or inline skates, animals, and personal music players are not allowed in the race. In consideration for your accepting my entry, I, for myself and anyone for whom I am acting waive and release the $5K Race, Twin City Track Club, Inc. and its officers, agents, and volunteers, the city of Winston-Salem and the Road Runners Club of America, all event sponsors, their representatives and successors from all claims or liabilities of any kind arising out of my participation in this event, even though that liability may arise out of negligence or carelessness on the part of the persons named in this waiver.

I grant permission to all the foregoing to use my image in photographs, motion pictures, recordings or any other record of this event for any legitimate purposes. I understand that this event does not provide refunds in the event of a cancellation, and by signing this waiver, I acknowledge that I have read this release of liability and assumption of risk agreement, fully understand its terms, understand that I have given up substantial rights by signing it, and sign if freely and voluntarily  without any inducement.

Must be signed by a parent/guardian if Participant is under age 18.
