In consideration of acceptance of this entry and my participation in this event, I, the undersigned, accept all risk of death, injury, and property loss or damage and assume full and complete responsibility for any injury or illness to my person or damage or loss to my property that may occur in conjunction with my participation in this event or while I am on the premises of this event. I state that I am in proper condition to participate in this event.

I, for myself and anyone entitled to act on my behalf, hereby release and agree to hold harmless The University of Texas MD Anderson Cancer Center and The University of Texas System and their Regents, officers, employees, agents, and representatives, and all other persons and entities associated with this event (including sponsors, volunteers, exhibitors, and vendors) (collectively, the “Event Parties”) from all loss, costs, damages, liabilities, claims and causes of action arising out of or related to my participation in this event or related activities (including, without limitation, any claim or cause of action based upon the sole or concurrent negligence of any of the Event Parties).

I hereby give my permission to the Event Parties to use, without compensation, for any purpose related to this event or any future event and in any medium (including broadcast, newspaper, magazine, brochure, Internet, or other media) my name, likeness and/or image (including any photographs, videotapes, motion pictures, or other recordings of me that are made during the course of this event or related activities).
