Please carefully review the following statements:

The Donate Life Legacy Walk (Event) is intended to bring people together to honor donors, enjoy music, food, and time outdoors, and will be held at the National WWI Museum and Memorial on September 12, 2026.  Participation in the Event may involve light walking and prolonged standing in a park-like setting. 

I represent and promise that I am in adequate physical and mental condition to safely participate in the Event and that I will adjust my participation to suit my own abilities. I understand it is my responsibility to determine what activities are suitable for me to participate in and that I will seek advice from a medical professional should I have any concerns. 

I understand that MTN has undertaken reasonable efforts to create a safe and enjoyable environment but ultimately cannot guarantee my safety during the event. I acknowledge that my participation involves risks that are inherent to outdoor community gatherings, including uneven walking surfaces, weather conditions, crowds, wheeled methods of transportation, the actions of other participants, and potential exposure to illnesses. I accept and assume these risks. 

I understand that in consideration of participating in the Event, I release, waive, and hold harmless MTN from any claims or damages that may arise or relate to my participation in the Event, including those resulting from negligence. I also agree to comply with all Event rules and instructions and acknowledge MTN’s right to remove any participant whose conduct is unsafe or inconsistent with the spirit of the Event. 

By registering for and/or attending this Event, I grant MTN permission to use my name, likeness, image, voice, and appearance in photographs, video, and audio recordings taken during the Event (Media) for educational, promotional, and informational purposes in any medium now known or later developed. I understand that I will not receive compensation for such use and waive any rights of privacy or publicity related to the Media. I also release MTN from any claims relating to the Media or use of the Media. 

I acknowledge that I have carefully read and understand this Release, and I am freely and voluntarily waiving legal rights.
