In consideration of Animal Friends Humane Society permitting me to participate in the Dog Jog and/or
5K (no animals are allowed in 5K) activities (collectively, the “Event”), I, on behalf of myself, my heirs,
executors, representatives, administrators, agents and assigns (collectively the “Releasors”) irrevocably
and unconditionally fully and forever waive, release and discharge Butler County, Ohio, the City of
Fairfield, Ohio, Animal Friends Humane Society (AFHS), and each party’s Waiver for agents, officers,
elected or appointed officials, directors, employees, and volunteers in their corporate and personal
capacities (collectively, the “Released Parties”), from any and all claims, demands, actions, causes of
actions, judgements, rights, fees, damages, debts, obligations, liabilities and expenses (including
attorney’s fees) of any kind whatsoever, whether known or not, whether a rising out of contract or
negligence, that Releasors may have or may ever have against the Released Parties arising out of, or in
any way related to my participation in this Event. I know that participating in a road race is a potentially
hazardous activity, and that I should not enter, run, walk or jog unless I and my animal are medically
able to do so and sufficiently trained. However, with this knowledge I have made a decision to
participate, and having done so, I hereby assume all risks associated with participating in this Event
including, but not limited to falls, contact with other participants and other animals, exposure to and/or
contracting a communicable disease such as COVID-19, the adverse effects of weather, traffic, and
course conditions. I acknowledge that all such risks are known by me, understood by me, and assumed
by me. In the event of any illness, injury or medical emergency arising to myself or my animal during the
Event, I hereby authorize and give my consent to the Event organizers / volunteers to secure any
medical treatment that they deem to be necessary (including, but not limited to treatment, medications,
medical transport, and hospitalization) from any medical professional, emergency medical technician,
hospital or clinic, and I agree that I will be fully responsible for the full payment of any charges for these
goods and services. I agree to abide by all decisions of any race official relative to my and my animal’s
ability to safely complete the run. I certify as a material condition to my being permitted to enter this
race that I am physically fit and sufficiently trained for the completion of this event. If I am participating
with my animal, then I also certify that he or she is physically fit and sufficiently trained for the
completion of the event. I understand that at any time during the Event, I can be asked to leave the
Event if I or my animal displays any inappropriate behavior, including but not limited to acts of
aggression that may be threatening to any other persons or animals at the Event, and if this happens
there will not be any refund of my entrance fee. I understand the Event allows only one animal per
person and that all animals must wear ID tags, be on a leash (no retractable leashes permitted), socialize
well with other dogs and people, be current on rabies vaccinations, and not be in heat. I understand no
animals are permitted in the 5K and that if I am participating in both the 5K and any other part of the
Event, I will have someone else care for my animal while I participate in the 5K. I further understand that
I am solely responsible for the actions of any animal that I bring to the Event. I understand the Animal
Friends Humane Society reserves the right, in its sole discretion, to refuse entry of any animal or person
based on safety or other concerns. This event follows the standard running industry policy: All entry fees
are non-refundable. AFHS reserves the right to postpone or cancel the event due to circumstances
beyond its control, including but not limited to a natural disaster or emergency, or as required to protect
the safety of participants, animals, staff, volunteers, sponsors and the public. No refunds will be issued
under these circumstances. AFHS reserves the right to change the details of the Event without prior
notice. I understand that my entry fee is nonrefundable and bib numbers are non-transferable.
Employees, agents or volunteers for AFHS, members of the press/media, or other persons who may
attend or participate in this Event may photograph, videotape or take statements. I grant permission to

all of the foregoing to use my photographs, motion pictures, recordings or any other record of this event
for any legitimate purpose. Covenant Not To Sue – I agree not to bring suit, action, proceeding or claim
against AFHS or any other Released Party for any dispute or dissatisfaction arising out of my
participation, or application to participate in this Event. By submitting this entry, I acknowledge (or a
parent or adult guardian acknowledges for all children under 18 years) that I have read the entire
Application, Waiver & Release form including the no refund policy, and that I agree to all of the Terms and Conditions contained herein.
