Located at 2750 Big Lots Parkway in Durant, Choctaw Wellness Center membership is available to Choctaw tribal members, Choctaw Nation associates/employees, CDIB cardholders of a federally recognized tribe, and their spouses and dependents up to age 26, as well as military, and their spouses and dependents up to age 26.
