The Care@Home Crusade 5K Walk/Run

The 5K Crusade is a charitable event and fundraiser that will be held Saturday, June 2, 2018 at the University of South Alabama on the "Swamp Loop" 5 K course.
On event day, registration will take place at the SGA Pavilion where a mini-Health Info tent, fun, snacks and festivities are planned from 10:00 AM until 2:00 o'clock in the afternoon.
the race will starting at 11:00 am 
Remembering the Elders in the Village (REV) is hosting this event to help generate funds and resources for patients who have great difficulty paying for non-covered home-care services, whose family members or caretakers need respite, and who may require Personal Care Services. The REV voucher program has been designed to help patients who have fallen behind on non-covered services or who can demonstrate an economic hardship by subsidizing a portion of the cost of their Home Care trip fees. College Students with valid ID: $10  School-aged Students (K-12) with valid ID: $5
Entry fees include a race shirt. No Shirt Option: subtract $3 from ticket cost
Awards: Top three Male and Female finishers will receive trophies. Top three male and female
finishers in each age group will receive medals: 9 and under, 10 —14, 15—25, 26—36, 37—47,
48—58, 59—69, 70 —80, 81 and above.

Post-Race Party: Refreshments will be available. Water will be provided at no cost.

I know that running a road race is a potentially hazardous activity that could cause injury or death. I should not enter and run unless I am medically
able and properly trained, and by my signature I certify that I am medically able to perform this event, am in good health and am properly trained. I
agree to abide by any decision of a race official relative to any aspect of my participation in this event, including the right of any official to deny or
suspend my participation for any reason whatsoever. I assume all risks associated with running or walking in this event, including but not limited to:
falls, contact with other participants, the effects of the weather, including high heat and/or humidity, traffic and the conditions of the road, all such
risks being known and appreciated by me. I understand that bicycles, skateboards, baby joggers, roller skates or blades, animals, and radio
headsets are not allowed in the race and I will abide by these guidelines. In consideration of Remembering The Children in the Village (RCV)
Remembering the Elders in the Village Program (REV) and the University of South Alabama, their sponsors and
agents, servants, representatives, licensees, contractors, successors, and assigns organizing and conducting the CARE@HOME CRUSADE 5K,
and allowing me to participate and run in said race/events, I hereby waive, release and discharge forever the said RCV, REV, University of
South Alabama, its trustees, officers, agents, servants, and employees, all sponsors, their representatives and assigns from any and all claims,
demands, right and causes of action whatsoever kind and nature arising directly or indirectly, from any and all known or unknown, foreseen or
unforeseen, bodily or personal injury, damage to property and the consequences thereof resulting from my running and/or participating in said
CARE@HOME CRUSADE 5K and covenant not to sue for any said injuries and/or damage. Further I grant full permission to any and all of the
foregoing to use any photographs, videotapes, motion pictures, recording or any other or any other record of this event for any purpose whatsoever.
I have read the above release and understand that I am entering the CARE@HOME CRUSADE 5K walk/run at my own risk.
