Members of the Hospice care team provide a comprehensive plan of care tailored to meet the needs of each patient and family. The team includes the patient’s own physician and a Hospice physician, registered nurses, licensed practical nurses, social workers, volunteers, and a chaplain.
Hospice care includes:
• 24 hour on-call nursing
• management of pain and other symptoms
• education of family members in care of patient
• personal care by licensed practical nurses and home health aides
• respite for primary caregiver by trained volunteers
• dietary consultation
• short term in-patient care
• prescribed medications
• medical equipment and supplies
• therapists as needed
• emotional and spiritual support
• bereavement services for the family
• referral services to other available community resources
