I attest and verify that I will participate in this event as a foot race entrant and that I am physically fit and have sufficiently trained for the completion of this event and that my physical condition has been verified by a doctor. Further, I hereby grant full permission to use my name and picture or other recording of me in this event for any purpose without obligation or liability to me.
