Memphis Youth Athletics Summer Program Waiver & Release of Liability
In consideration of you accepting this registration, I, the parent or legal guardian of the participant (the "Camper"), intending to be legally bound, do hereby waive, release, indemnify, and hold harmless the Camp Director, Memphis Youth Athletics, the University of Memphis, and all of their respective agents, staff, sponsors, representatives, volunteers, contractors, affiliates, and employees from and against any and all claims, demands, causes of action, damages, liabilities, losses, costs, or expenses arising out of or related to the Camper’s participation in the camp, including but not limited to any injuries to the Camper or damage to personal property occurring before, during, or after camp participation. I recognize, intend, and understand that this release and indemnification agreement is binding on myself, the Camper, and our heirs, executors, administrators, and assigns.
I understand that participation in a youth summer camp involves a variety of physical and recreational activities, including but not limited to running, games, outdoor play, sports, crafts, field trips, instructional activities, and activities occurring on or around the University of Memphis campus and facilities. I acknowledge that these activities carry certain inherent risks, including but not limited to trips and falls, weather-related injuries, interactions with other participants, insect bites, sun exposure, equipment-related injuries, and transportation-related risks during off-site trips. I certify that the Camper is physically able to participate in all camp activities and that a licensed medical professional has cleared them for participation if necessary.
In the event of an illness, injury, or medical emergency arising during camp participation, I hereby authorize and give my consent to the Camp Director and/or their designees to secure from any licensed hospital, clinic, physician, or emergency personnel any treatment deemed necessary for the Camper’s immediate care. I understand and agree that I will be fully responsible for the cost of any medical treatment or services rendered, including but not limited to medical transport, medications, emergency care, and hospitalization.
As it relates to participation in the camp, I agree to follow any current public health guidelines, including those provided by the Centers for Disease Control and Prevention (CDC), as well as any additional health and safety protocols implemented by Memphis Youth Athletics, the University of Memphis, or camp organizers related to COVID-19 or other communicable illnesses.
Furthermore, I grant permission to Memphis Youth Athletics, The University of Memphis, and its representatives to use the Camper’s name, voice, likeness, and image in any photographs, videos, publications, social media content, websites, advertisements, or other promotional materials created during camp participation for legitimate educational, promotional, and organizational purposes without compensation.
I understand that all camp registration fees are non-refundable. The camp reserves the right to postpone, reschedule, modify, or cancel any camp days, sessions, field trips, or activities due to weather, emergencies, public health orders, facility issues, insufficient enrollment, or other circumstances beyond its control, and no refunds will be issued in such cases. Program details, schedules, activities, and locations are subject to change without notice.
Memphis Youth Athletics Zero Tolerance Policy on Fighting, Bullying, and Abuse
At Memphis Youth Athletics, the safety and well-being of every child in our care is our highest priority. We are committed to maintaining a positive, respectful, and inclusive environment where all campers feel safe, supported, and encouraged.
As such, Memphis Youth Athletics maintains a zero tolerance policy for any form of fighting, bullying, physical aggression, harassment, intimidation, or verbal abuse.
Policy Guidelines
-Fighting, whether physical or verbal, will not be tolerated under any circumstances.
-Bullying, including teasing, exclusion, intimidation, harassment, threats, or any repeated harmful behavior—whether in person, online, or through electronic communication—is strictly prohibited.
-Physical or verbal abuse directed toward another camper, staff member, volunteer, university personnel, or visitor is grounds for immediate disciplinary action.
-Destruction of property, inappropriate language, threatening behavior, or unsafe conduct may also result in disciplinary action or removal from camp.
Consequences
-If your child is involved in any such incident, you will be contacted immediately after the situation has been addressed.
-Your child may be sent home for the remainder of that day.
-Depending on the severity of the incident, or if your child is a repeat offender, they may be suspended or removed from the camp for the remainder of the session or the full summer without refund.
-This policy exists to protect all participants and ensure a safe and enriching experience for everyone. By registering for camp, you acknowledge and agree to abide by this policy.
By submitting this registration, I acknowledge that I have carefully read, fully understand, and voluntarily agree to the terms of this waiver, release of liability, indemnification agreement, medical authorization, media release, and camp policies, including the no-refund policy. I certify that I am the legal parent or guardian of the participant and am authorized to sign this agreement on their behalf.
