In consideration of my entry, I intend to be legally bound, for myself, my heirs, executors, and administrators, waive, release, and forever discharge any and all rights and claims which may accrue to me against the directors and volunteers of the run, all sponsors, CASA DeKalb County, Inc., National CASA, Illinois CASA, the County of DeKalb, Sycamore Park District, Sycamore School Dist #427, the City of Sycamore, and RunSignup.com for any and all accidents or injuries incurred by me at this event. I grant permission and give my consent for the use of photos, videos, and electronic media images. I state that I am aware of the problems that could be encountered including the street traffic, heat, humidity, crowds, color from packets/spraying devices and from animals. I further state I have trained adequately and am in suitable athletic condition to compete in the race I have entered. I understand if the race is canceled due to unforeseeable circumstances, such as inclement weather, there will be no refunds.
SYCAMORE PARK DISTRICT WAIVER AND RELEASE OF ALL CLAIMS: Please read this form carefully and be aware when registering yourself, your child, child/ward for participation in the above program(s) you will in waiving and releasing all claims for injuries you or your minor might sustain arising out of above program(s). I recognize and acknowledge that there are certain risks of physical injury program(s), and I agree to assume the full risk of any injuries (including death), damages, or loss, regardless of severity, which I or my minor child/ward, arising out of, connected with, or in any way associated with the activities of the program(s).
I do hereby fully release and discharge the Sycamore Park District and its officers, agents, servants and employees from any and all claims resulting from injuries, damages, and losses sustained by me or my minor child/ward arising out of, connected with, or in any associated with the activities of the above program(s). I further agree to indemnify and hold harmless and defend the District and its officers, agents, servants, and employees from any and all claims resulting from injuries, damages and losses sustained by me or my minor child/ward arising out of, connected with, or in any way associated with the activities of the above program(s).
By submitting this entry, I acknowledge (or a parent or adult guardian for all children under 18 years) having read and agreed to the above release and waiver including the no refund policy.
