BEER MILE WAIVER AND RELEASE OF LIABILITY AGREEMENT

PLEASE READ CAREFULLY BEFORE SIGNING

I acknowledge that participating in the 2026 Ride for Levi Beer Mile, which includes the consumption of alcohol and running, is a potentially hazardous activity. I certify that I am physically fit, have not been advised otherwise by a qualified medical professional, and am voluntarily participating in this event with full knowledge of the risks involved.

I fully understand and acknowledge that:

Alcohol impairs physical coordination and judgment.

Running while under the influence of alcohol carries inherent risks, including injury or death.

I am responsible for knowing my own limits and for ceasing participation if I feel unsafe.

Participation is entirely voluntary and at my own risk.

In consideration of being allowed to participate, I hereby release, waive, and discharge the Town of Montrose, Dane County Parks, Paoli Pub, all owners, officers, directors, employees, volunteers, sponsors, affiliates, and all other participants from any and all liability, claims, demands, or causes of action related to any loss, damage, injury, or death that may be sustained by me while participating in or traveling to/from the Event.

I further agree to indemnify and hold harmless the released parties from any loss, liability, damage, or costs they may incur due to my participation.

I affirm that I am 21 years of age or older and legally permitted to consume alcohol.

PHOTO RELEASE: I grant permission to the Event organizers to use photographs, video, and/or audio of me taken at the Event for promotional purposes without compensation.
