MONA CITY TOURNAMENT/ACTIVITY PARTICIPATION WAIVER 
LIABILITY WAIVER: I acknowledge that participation in the Mona City 
Day of Service Fun Run & 5K may involve hazards and 
risks, including bodily injury. I agree to assume these risks, and I waive, release, and forever 
discharge Mona City and its officers, employees, agents, representatives, and volunteers from 
any and all claims for injury, illness, damage, or loss (including death) arising from my 
participation. I agree to defend and indemnify Mona City for any related claims or lawsuits. This 
release is intended to be as broad as permitted under Utah law and is binding on my heirs and 
legal representatives. 

PARENTAL CONSENT: If a participant is under 18, their parent/guardian consents to the child's 
participation, accepts the risks described above on the child's behalf, and releases Mona City 
from related liability, including for first aid rendered during the event. 

PHOTO & RECORDING RELEASE: I grant Mona City permission to photograph or record me 
(or my child) during the event, and to use those images/recordings for promotional purposes 
(website, social media, print, event advertising) without compensation to me. 
By signing below, you confirm that you have read, understood, and agree to the Liability Waiver, 
Parental Consent (if applicable), and Photo & Recording Release on behalf of yourself and every 
individual listed on this form.
