WAIVER AND RELEASE OF CLAIMS
By virtue of attending and/or participating in the YV5K, sponsored by Youth Villages (the
“Company”), and in consideration of being allowed to attend said event, and for other good and valuable consideration, said participant (the “Participant”) understands, acknowledges, represents, warrants and agrees as follows, with the knowledge that the Company will rely on same:
1. Participant desires to participate in the YV5K  offered by the Company.
2. Participant is in good health and suffers from no minor or serious physical or mental injury, illness or disability that would make Participant especially susceptible to injury or disability while performing any activity contemplated by this Waiver and Release of Claims.
3. Participant fully comprehends and accepts all of the risks associated with the YV5K, I,
for myself, my heirs, personal representatives or assigns, employees and affiliates, do hereby release, waive, discharge, and covenant not to sue Youth Villages, their affiliates, direct or indirect, officers, employees, businesses and agents from liability from any and all claims resulting in personal injury, accidents or illnesses (including death), and property loss arising from participation in the YV5K.
4. Participation in YV5K carries with it certain inherent risks that cannot be eliminated
regardless of the care taken to avoid injuries.
5. Participant grants to Company, including Company’s successors and assigns, the right and authority to use Participant’s name, likeness, photograph and/or picture from YV5K for any and all commercial or non-commercial purposes known or later developed in perpetuity throughout the universe without further obligation or compensation to Participant.
