ACOG on the Move! 

WAIVER 

In consideration of being permitted to participate in the ACOG on the Move! 5k/1M/Run/Walk/Sleep-In, I agree to assume all risks inherent in participation in such program, whether they are apparent to me or not.  I certify that I am in good physical health and fit to participate, sufficiently trained for the completion of this event and that a licensed Medical Doctor has verified my physical condition. 

I acknowledge that participation carries an inherent risk of injury to my person and damage to my property.  I hereby waive and release, for myself and for my heirs and assigns, any and all claims causes of action or liabilities which may hereaft accrue against Blue Cheetah Sports Timing, LLC, the Event Director, RunSignup.com, American College of Obstetricians & Gynecologists (ACOG), ACOG Foundation, subcontractors, volunteers, and any and all sponsors, their representatives and successors (collectively the “Hosts”), that may arise as a result of my participation in the ACSM 2026 5k, including any and all claims for personal injuries caused by the negligence of the Hosts.

I grant full permission to any and all organizations which obtain photographs, motion pictures, videotapes, recording of any other record of the event to use them for any purpose whatsoever, without monetary payment to me.

I agree to abide by all decisions of any race official relative to my ability to safely complete the run. 

In the event of an illness, injury or medical emergency arising during the event I hereby authorize and give my consent to the Event Director to secure from any accredited hospital, clinic and/ or physician any treatment deemed necessary for my immediate care. I agree that I will be fully responsible for payment of any and all medical services and treatment rendered to me including but not limited to medical transport, medications, treatment and hospitalization. 

Further, I grant permission to all the foregoing to use my name, voice and images of myself in any photographs, motion pictures, results, publications or any other print, videographic or electronic recording of this event for legitimate purposes. 

I understand and agree that: Hosts reserve the right to postpone or cancel the event without notice for any reason, including due to circumstances beyond Hosts’ control such as a natural disaster or emergency or as required to protect the safety of participants and staff; and all paid entry fees are non-refundable regardless of whether the event is cancelled or postponed; bib numbers are non-transferable. 

By my signature below, I acknowledge for myself and as the parent or adult guardian for any children under 18 years which I am registering, that I have read and agreed to all the terms set forth above, including the no refund policy and the waiver of liability. 

This section to be read and signed by parent/legal guardian if Participant is a minor:

As the parent/legal guardian of the above-named minor participant, I hereby waive and release on behalf of my minor child, any and all claims, and causes of action, or liabilities which may hereafter accrue against the Hosts, by reason of my child’s participation in said program, including any and all claims for personal injuries caused by the negligence of the Hosts. In addition, I accept full responsibility for the care and supervision of my child during the above-described event.
