I understand that dance is a physical activity and that there is an inherent risk of injury in participating in dance. I give approval for the enrolled student to participate in all aspects of Charles River Ballet Academy. I will not hold Charles River Ballet Academy or any of its teachers, students, or volunteers responsible for any injury to the enrolled student while participating in its classes, rehearsals, or performances. I also give approval for photos of the enrolled student to be used in CRBA ads, programs, poster, DVD cover, and website. I understand there will be no refunds or credits.

By checking this box, I agree to the waiver and that I am 18 or older, or that I have the authority to register these participants and agree to the waiver for them, and agree to the Privacy Policy.
