I understand that I will pay here for Ballet Together. I understand that dance is a physical activity and that there is an inherent risk of injury in participating in dance. I give approval for the enrolled student to participate in all aspects of Charles River Ballet Academy. I will not hold Charles River Ballet Academy or any of its teachers, students, or volunteers responsible for any injury to the enrolled student while participating in its classes, rehearsals, or performances. I also give approval for photos of the enrolled student to be used in CRBA ads, programs, poster, DVD cover, online platforms, and web site. I understand that tuition is non-refundable and no credits will be given for withdrawals.
