I understand I am registering for a free trial class and/or open house at Charles River ballet Academy and that Sept 1 and 12 events are at Studio B (1154 Great Plain Ave Needham) and that Sept 9 events are at Studio A (23 Dedham Ave Needham).  I understand my family is welcome to attend Open House on Wed. Sept. 9 at Studio A from 4-5pm and Sat. Sept. 12 from 10-11am regardless if my child is attending a free trial that day. I understand that dance is a physical activity and that there is an inherent risk of injury in participating in dance. I give approval for the enrolled student to participate in all aspects of Charles River Ballet Academy. I will not hold Charles River Ballet Academy or any of its teachers, students, or volunteers responsible for any injury to the enrolled student while participating in its classes, rehearsals, or performances. I also give approval for photos of the enrolled student to be used in CRBA ads, programs, poster, online platforms, and website.
