I understand that participation in the Company's Wellness Challenge is voluntary. The challenge encourages participants to engage in physical activities such as walking, running, cycling, swimming, fitness classes, and other forms of exercise to track mileage and support personal wellness goals.

I acknowledge that physical activity involves certain inherent risks, including but not limited to falls, strains, sprains, overexertion, and other injuries that may occur during participation. I understand that it is my responsibility to determine whether I am physically able to participate in any activity and to consult a healthcare provider if I have questions regarding my health or ability to participate safely.

By participating in this Wellness Challenge, I voluntarily assume all risks associated with my participation and release and hold harmless the Company, its officers, directors, employees, volunteers, sponsors, and representatives from any and all claims, liabilities, demands, actions, or causes of action arising out of or related to any injury, illness, loss, or damage that may occur as a result of my participation.

I agree to participate in a safe and responsible manner, follow any applicable safety guidelines, and select activities that are appropriate for my individual fitness level and health condition.

By registering for or participating in this Wellness Challenge, I acknowledge that I have read, understood, and agree to the terms of this waiver and release.
