I acknowledge receipt of the following raffle‑related disclosures, provided by Lightning Warriors Hockey Program, Inc. (“LWHP”) in compliance with section 849.0935, Florida Statutes, in connection with any raffle or similar drawing conducted in association with the Event: (a) Rules Governing the Raffle – The raffle drawing will take place in person, in public view, by drawing electronic entries following the completion of the Event each year. Winners need not be present to win and will be notified using the contact information provided at entry. (b) Organization Information – Lightning Warriors Hockey Program, Inc. is a licensed and certified 501(c)(3) nonprofit organization, with its principal place of business located at 8030 111th Terrace E., Parrish, Florida 34219. (c) Source of Prize Funds – Prizes are funded through donations from local and national supporting organizations and individual donors. (d) Date, Time, Place, and Description of Prizes – Winners will be selected at the Event location following completion of the annual 5K. Prizes may include such things airline tickets, sporting event tickets, autographed hockey memorabilia, and other promotional items donated by sponsors. Updated prize information may be posted at https://x.com/warriors5k. (e) No Purchase Necessary – No purchase or contribution is necessary to enter or win the raffle. A raffle entry may be obtained without donation by emailing run@lwhp.org during the applicable entry period and following the instructions provided to complete an official entry form and submit any required information.
