LIABILITY WAIVER
In consideration of being permitted to participate in any club organized events, group training runs or social events (the “Club Programs”) of Columbus Westside Running Club, Inc., (the “Club”), I, for myself and anyone entitled to act on my behalf, do hereby forever release, waive, covenant not to sue and discharge the Club (including its members, officers, employees and agents), the Road Runners Club of America, and all Club sponsors, from all claims or liabilities of any kind whatsoever including, without limitation, those arising at any time from the negligence of the Club or any of its members, officers, employees or agents, arising out of my participation with the Club. Further, I hereby agree to indemnify, defend and hold harmless the Club and its members, officers, directors, employees and agents from any and all claims, actions, suits, costs, expenses, damages and liabilities brought, asserted, incurred or arising from or as a result of the Club Programs or participation with the Club, and to the extent caused by, arising out of, or relating to my actions. Participation in any running program carries with it certain inherent risks that cannot be eliminated regardless of the care taken to avoid injuries. I know that running in and volunteering for organized group runs and races are potentially hazardous activities which can cause or lead to serious physical injury or death. I assume these risks and all other risks associated with being a member of this club which may include: falls, contact with other participants, other users of roads and trails, the effects of the weather, including high heat and/or humidity, traffic, including motor vehicles, bicycles, skates and skateboards, strollers, animals and the conditions of the trails and roads, all such risks being known and appreciated by me. I will not participate in any club organized events, group training runs or social events, unless I am medically able and properly trained, and by my signature, I certify that I am medically able to perform all activities associated with the club and am in good health, and I am properly trained. I hereby assert that my participation and agreement to this release is voluntary; that I have read and understand this release; and that I knowingly assume all such risks. I understand that the Club is relying upon this release as a condition of my participation, and that it would not agree to permit my participation in the absence of this release. I expressly agree that the foregoing waiver and assumption of risks is intended to be as broad and inclusive as is permitted under the laws of the State of Ohio. If any portion thereof is held invalid or unenforceable, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

CODE OF CONDUCT
I agree that I am a member of Columbus Westside Running Club (CWRC) and that all members shall not discriminate against any individual with regard to race, creed, color, national origin, gender, sexual orientation, physical condition, or age. Additionally, CWRC members shall strive to represent him or herself and CWRC respectfully in all situations, and treat others with kindness and respect in all dealings as a member. I agree to abide by all rules established by the club, including the right of any official to deny or suspend my participation for any reason whatsoever. I grant permission to use my photographs, motion pictures, recordings or any other record for any legitimate promotional purposes for the club.
