In consideration of the acceptance of this entry, I waive and forever release for myself and my heirs, executors, administrators, and assignees any and all rights and claims for damages or injuries that I may have against the LA COURSE 5K Race/Walk event (“Event”), The French American School of Rhodes Island (“FASRI”), the City of Providence, the Event Director, True North Running Company LLC, RunSignup.com, and all of their agents, representatives, and employees; all volunteers assisting with the Event; and all sponsors of the Event for any and all injuries that may occur to me, my minor child, if applicable, and/or my personal property as a result of my participation in the Event.
I know that participating in a road race is a potentially hazardous activity. I assume all risks associated with participating in the Event including, but not limited to, falls, contact with other participants, the effects of weather, traffic, and course conditions, and I waive any and all claims which I might have based on any of those and other risks typically found in participating in a road race. I acknowledge all such risks are known and understood by me. I should not participate in the Event unless I am medically able to do so. I certify as a material condition to my being permitted to enter the Event that I am physically fit to participate in the Event and that my physician has verified my physical condition. I agree to abide by all decisions of any race official relative to my ability to safely complete the Event.
The Event follows the standard running industry practices: All entry fees are non-refundable. FASRI reserves the right to postpone or cancel the Event due to circumstances beyond our control such as a natural disaster or emergency or as required to protect the safety of participants. I also understand that the event may change due to a decision by the City of Providence. No refunds will be issued under these circumstances. FASRI reserves the right to change the details of the Event without prior notice. I understand that all entry fees are nonrefundable and bib numbers are nontransferable.
In the event of an illness, injury or medical emergency arising during the Event, I authorize and give my consent to the Event Director to secure from any hospital, clinic, and/or physician any treatment deemed necessary for my immediate care. I agree that I will be fully responsible for payment of any and all medical services and treatment rendered to me including, but not limited to, medical transport, medications, treatment, and hospitalization.
I also grant permission to FASRI to store, use, license and reproduce the use of my name, likeness, voice, image, motion pictures, recordings, or photographs of me, in any media or in any manner, including any electronic media for any legitimate commercial or business purposes, including advertising purposes.
By submitting this entry, I acknowledge that I am 18 years of age or older, or that I am the parent or legal guardian of a child under the age of 18 who I am registering for the Event.
