Paperwork and Fees

MHS ATHLETICS PAPERWORK:   Everyone must complete the entire athletics paperwork packet and pay the $50 insurance in order to participate in any sport (including conditioning) at MHS. Once you complete the paperwork packet and pay the insurance, you are cleared for all sports for the entire school year.

2021-22 athletic paperwork (all 7 pages)
$50 insurance may be paid by cash, money order, or check payable to Manatee High School with a parent phone number on top of the check and student name on the memo line. If paying by cash, please write student name on envelope. All paperwork and insurance should be turned in to your coach.
If you completed the paperwork packet and insurance for a fall or winter sport, please check with your coach as you may already be cleared to participate.


TRACK & FIELD TEAM FEE:  Our team has a $40 per student-athlete team fee each year.  This goes towards our many costs of meet entry fees which range from $200-$250 per meet, team t-shirts, snacks at meets, transportation of $300-$500 per meet, etc.

You may pay your team fee online by selecting your primary event group from the membership levels on the Club Info (home) page or the Sign Up page.  You may also pay offline by cash or check - please see details in the membership levels on the Club Info (home) page. There are no refunds if you quit the team.


All paperwork, insurance, and team fees must be satisfied in order to be issued a uniform.
